There is also the option of a fee per service plan. If your insurance company is not listed, but they provide “out-of-network” services, we will be happy to provide you with the necessary documentation to submit to your insurance carrier so that you may obtain reimbursement for the per-visit fee.
A: If you are utilizing your health insurance, rather than the fee per service plan, then each visit will be billed to your insurance carrier. Your office visit co-pay will be due at the time of service.
A: It is unlikely that the annual fee will be reimbursable. However, certain Health Savings Account (HSA) and Flexible Spending Account (FSA) plans may possibly allow for reimbursement for all or part of the fees for physician services. Members should consult with their human resources representatives at their place of employment, or their HSA or FSA plan managers.
